New Patient Packet
Feel free to download and print the new patient packet to bring to your first appointment.
If your insurance company requires an insurance referral from your primary care physician, please submit it prior to your appointment. Our office requires a referral directly from your doctor, regardless of insurance. This referral must include your most recent and relevant office notes, demographics, a copy of your insurance card or authorization from workers' compensation, as well as any diagnostic study reports and any previous pain management records.
Virginia Brain and Spine Center does not provide narcotic medication management services to our patients. If you require narcotic medication management please consult your primary care to obtain a referral that will better suit your needs.
All medication refills are done during working hours on Monday through Thursday only. You may have your pharmacy call directly to request a medication refill. Please allow two working days for the prescription to be processed. If you need a new written prescription, please allow 5-7 business days for the prescription to be processed. We are unable to refill prescriptions after hours so allow enough time before your prescription runs out.
All patients should arrive 15 minutes prior to their appointment time to complete any necessary paperwork. If you arrive 15 minutes after your appointment time, you may be rescheduled. Please provide 24 hours notice prior to your appointment for cancellations, this allows us to better serve our patient population and avoid charging for missed visits.
Virginia Brain & Spine Center, Inc complies with HIPAA privacy standards, and will treat your medical records confidentially, consistent with our Notice of Privacy Practices. If you are being seen for a work related injury and are covered under a Workers' Compensation claim, we will seek payment from that plan as long as you provide us with the necessary claim contact information to allow us to obtain authorization in advance of your appointment. If your workers' compensation plan denies payment for services rendered, you are responsible for payment in full. If you have any questions regarding a workers' compensation claim, you may contact our billing department at (540) 450-0072.
