Appointments:
If you are a referring provider and want to refer a patient to us please fill out the referral form listed under either the Neurosurgery or Pain Management tab (select the specialty you want your patient referred to then click on the tab Consultations and the referral information will be listed)

If you are a new patient please download the new patient packet found within the Consultations tab in either Neurosurgery or Pain Management (whichever group you have your appointment set with) and fill that out prior to your first visit. Please notify our office if you are taking any medication that would “thin” the blood (examples: Coumadin/warfarin, Plavix/clopidogrel, Ticlid/ticlodipine, heparin, Lovenox/enoxaparin, Arixtra/fondaparinux).

Emergencies:
Please call 911 or go to your nearest emergency room.
