Appointments
If you are a referring provider and would like to refer a patient to VBSC please fill out the appropriate referral form for Pain Management or Neurosurgery (please select a specialty and you will be brought to the consultation page).

If you are a new patient please download the new patient packet for Neurosurgery or Pain Management (whichever group you have your appointment with) and have that completed prior to your first visit.  Please notify our office if you are taking any medication that would “thin” the blood (examples: Coumadin/warfarin, Plavix/clopidogrel, Ticlid/ticlodipine, heparin, Lovenox/enoxaparin, Arixtra/fondaparinux).
